The Kidney Function Clinic Clinician provides patient and family education, maintains patient statistics and maintains and develops clinical practice standards and documentation standards for patients with progressive renal insufficiency. The Kidney Function Clinic Clinician, provides leadership to the interdisciplinary team and co-ordinates clinic activities in the absence of the Full- time Clinician. As a key member of the interdisciplinary team, the Clinician facilitates communication amongst all interdisciplinary team members.
- Performs clinical assessments and treatments for clinic patients
- Provides follow up telephone assessments and consultations with patients or family members as necessary
- Reviews laboratory values with patient and family at each visit
- Provides education and information to patients and their families about kidney function, renal insufficiency and its symptoms
- Provides education and information about renal replacement therapy options
- Co-ordinates peer support visits between clinic patients and volunteers
- Collaborates with Hemodialysis and Peritoneal Dialysis in ensuring a seamless transfer of patients to these modalities.
- Co-ordinates referrals to outside institutions for patient care; transplants, surgeries, and referrals to other physicians
- Maintains professional development by attending or participating in seminars and conferences relevant to the care of the patient with chronic kidney disease and participates in ongoing continuing education initiatives
- Maintains a current knowledge base regarding ” best practice” and relevant, evidence-based research regarding care of the patient with chronic kidney disease
Qualifications
- Current registration in good standing with the College of Nurses of Ontario
- Demonstrated clinical expertise in Hemodialysis and vascular access required
- Demonstrated clinical expertise in peritoneal dialysis preferred
- CNA Certification in Nephrology required
- BScN, or in progress
- Understanding of the principles of adult education and demonstrated ability to apply those principles to educating Kidney Function Clinic patients
- Previous experience in the area of patient education an asset
- Demonstrated initiative and commitment to continuing professional development
- Demonstrated skills in leadership and organization
- Excellent communication skills
- Satisfactory employment and attendance record
