Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityThe Health Care Social Worker reports to the Program Manager and as a member of a multi-disciplinary team, provides a diverse range of interventions to clients and their families enabling them to achieve their optimal level of functioning and health. These interventions include: psychosocial assessments, adjustment therapy, supportive therapy, crisis counseling, resource counseling, discharge planning and case management. The Health Care Social Worker is actively involved in advocacy, education, program planning, consultation and collaboration on behalf of individuals/groups within the institution and community. The role is multifaceted and dynamic requiring the Health Care Social Worker to work autonomously as well as collaboratively.The Social Worker delivers programming with other members of the Nova Scotia Collaborative Inflammatory Bowel Disease Program (NSCIBD). The Social Worker may be required to provide services throughout the Rehabilitation and Supportive Care Services portfolio.About YouWe would love to hear from you if you have the following:

  • Master’s Degree in Social Work from a recognized university
  • Registered or eligible for registration with the NSCSW
  • 2 years of clinical experience as a clinical Social Worker required
  • Knowledge of the fundamentals of human behaviour including systems theory, family dynamics, group dynamics, and theories of behaviour change
  • Awareness of local community resources and agencies would be an asset
  • Valid Nova Scotia Driver’s License and access to a reliable automobile
  • Experience working with patients with complex chronic health conditions preferred
  • Experience co-facilitating education sessions with other multi-disciplinary team members preferred
  • Competencies in other languages an asset, French preferred
