Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityPlease note: This position requires the successful incumbent to strictly adhere to Integrated Chronic Care Services (ICCS) scent-free free policy.The Clinical Dietitian at Integrated Chronic Care Services is an integral member of the interdisciplinary health professional team and is responsible for the nutrition care, disease management and education of patients living with chronic conditions. The Dietitian initiates a cost-effective nutrition therapy comprised of patient assessment, the development and implementation of a patient-centered, goal-focused care plan, with ongoing evaluation. Nutrition care plans are outcome-focused, formulated to optimize both clinical and fiscal outcomes, and emphasize health promotion and disease prevention. The Clinical Dietitian is responsible for the continuum of care of patients with effective, evidence-based strategies to self-manage disease.About YouWe would love to hear from you if you have the following:
- Bachelor degree in Nutrition from an accredited institution is required
- Licensure by Nova Scotia Dietetic Association required
- Completion of Dietetic Internship accredited by Dietitians of Canada
- Membership in Dietitians of Canada required
- Two years of related experience in an adult health care facility is preferred
- Specialty Practice Certifications such as Certified Diabetes Educator (CDE), certification for the Diabetes Management Centre, and Certified Nutrition Support Dietitian (CNSD considered an asset.
- Access to reliable transportation for travel for work
- Competencies in other languages preferred; French an asset
